Paperwork, Paperwork, Paperwork... Boy Scouts, including Troop 104, use a wide range of forms to administer a safe and efficient program. Many of the National BSA forms are now available on our web site as well as several forms used by Troop 104. The forms are in PDF format, so you will need Adobe Reader to open them. Troop 104 Permission Form - parents are required to sign a permission form for all troop activities. Troop 104 Weekend Menu Planner(PDF) - Filled out for each Weekend event by each Patrol and checked by an adult for nutritional content. Troop 104 Weekend Menu Planner(EXCEL) National Tour Permit - required for all groups traveling to areas 500 miles or more one way from home area (local council camp excepted), or crossing national boundaries into the territory of other nations. Local Tour Permit - required to be filed with local council service center two weeks in advance of scheduled activity for trips of less than 500 miles. Medical Exam, Class 1 and 2 - required for Scouts and adults under the age of 40, with certain exceptions. Please check the form for specifics. Medical Exam Class 3 - required for high-adventure activities, athletic competition, and world jamborees. Class 3 exams are only valid for 12 months and must be completed by a licensed health-care practitioner. This form is to be used by adults over 40 for all activities requiring a physical examination and applies to all Wood Badge participants/staff regardless of age. Troop Meeting Plan - this form is used as a program outline to create an agenda for organized troop meetings. Campfire Program Planner - this form is used as a program outline for excting and diverse campfire programs. BSA Lifeguard - Scouts wanting to become BSA certified Lifeguards need this form for their lifeguard training program. First Class Tracking Sheet - this form is used as a program guide to ensure Scouting activities meeting advancement needs of First Year Scouts. 50-Miler Award - The 50-Miler Award is presented to each qualifying individual for satisfactory participation in an approved trip for five consecutive days by canoe, boat, or by foot of 50 miles or more. Merit Badge Counselor Application - The Merit Badge Counselor Application AND an adult application are required for anyone wishing to become an approved merit badge counselor.
