Boy Scouts, including Troop104, use a wide range of forms to administer a safe and efficient program. Many of the National BSA forms are now available on our web site as well as several forms used by Troop 104. The forms are in PDF format so you will need Adobe Reader to open them. Parent Survey - Parent Survey See positions of interest available within the Troop and District.
Troop 104 Permission Form - Parents are required to fill out and sign this form (this form is good for 1 year)
Health Form (New for 2010) - New Health form valid now and required after December 2009. Required for All Scouts and Adult Leaders. Good for 1 year.
Troop 104 Weekend menu Planner (PDF) - Filled out for each weekend event by each patrol and checked by an adult leader for nutritional content.

Personal Overnight Camping Essentials -What should the scout take on an overnight campout
National Tour Permit - Required for all groups traveling to areas 500 miles or more one way from home area.(local council camp excepted), or crossing national boundries into the territory of other nations.
Local Tour Permit - Required to be filed with local council service center 2 weeks in advance of scheduled activity for trips over 50 miles and less than 500 miles.
Troop Meeting Plan - This form is used as a program outline to create an agenda for organized troop meetings.
Campfire Program Planner - this form is used as a program outline for exciting and diverse campfire programs.
First Class Tracking Sheet - this form is used as a programm guide to ensure Scouting activities meet advancement need for First Year Scouts.
50-Miler Award - The 50-Miler award is presented to each qualifying individual for satisfactory participation in an approved trip for five conconsecutive days by canoe, boat, or by foot of 50 miles or more.
Merit Badge Counselor Application - The Merit Badge Counselor Application and an Adult aplication are required for anyone wishing to become an approved merit badge counselor.
